What i get from watching the video is him talkin to those who are throwing their life away by not caring what they do to their body.
Life Lined up on the mirror dont blow it - referring to those who snort coke and end up overdosing
the razor blade around his neck - like D mentioned, symbolizing those who took their lives with a razor
in the third verse he mentions looking like his dad and how he looks back on the past - meaning he is glad to have given the white cup up, and getting sober,(i believe he still smokes but who has ever died from weed?) and he see's his moms smile, thats a blessing.
