No he's talking about his uncle in the second verse. His uncle had a great influence on him. He mentions it on other songs as well like fear. His uncle also just passed this summer. And I'm pretty sure thats grandma on the phone, her son would be uncle.

The women on the phone was his grandma & the surgery his mom had was on her spinal cord I read somewhere that he said before he was able to afford the operation her back caused her lots of trouble & gotten her into usin painkillers & that she was struggling to even walk

A bone in his mothers spine was deteriorating which would ultimately leave her paralyzed. she has since had the surgery and is doing well
