A third wheel is basically a person who is the awkward-tag along, mostly in situations with a couple. Like, if you were planning to go out with your GF/BF but felt bad and didn't want to leave one of your friends out, so you invited them a long, that friend would be the third wheel. Third wheels make it awkward to display affection/discuss certain things.

In situations where it's not an established couple though, a third wheel is basically a massive cockblock which generally prevents you from making a move on whoever you are trying to make a move on.
