Graphics Request Form

Here is the request form for when requesting a graphics from an artist.

You must fully and properly fill out this request form:


Type of Image: Signature, cover, avatar etc.
Image: provide a link to an image you want used in the graphic (example: http://i356.photobuc...062690-Full.jpg)
Main Text: what would you like the title of the graphic to be... (generally your name)
Subtext (if any): (any quotes or awesome phrases you want added)
Additional Comments: Specific artist? anything you would like to add? specific size, colors, style, etc?
